Yes! Every form auto-saves to your browser. If you close the tab and come back later (on the same device and browser), your answers will still be there. Just open the form again to continue.
That's completely normal — feel free to sit with your child and help them understand the questions. The student forms are designed to capture YOUR CHILD'S voice, so let them answer in their own words, but supporting them is part of the partnership.
Only your tutor and Ebata L.U.M.A. Studio leadership. All information is kept strictly confidential, in alignment with FERPA principles. We never share your information with third parties without your explicit consent.
Ideally, complete them within your first week so we can prepare a personalized plan before your first session. If you need more time, just let us know — we'll work with your schedule.
Your tutor will review all the forms and create a personalized learning plan with measurable goals (Form 10). At your first session, we'll discuss the plan together and adjust based on your input. You'll receive your first monthly progress report at the end of Week 4.
Absolutely. As your child grows and changes, your needs may evolve. Just contact us at ebatalumastudio@gmail.com and we'll resend any form you'd like to update.
